​Faculty and Staff Vaccination Clinic – Tuesday, September 30

Time: 9:00 AM – 11:00 AM
Location: Room M129, Riverfront (Main) Campus

Jewel-Osco Pharmacy will be onsite to provide a variety of vaccines to faculty and staff.

If you are interested in receiving a vaccine, please email me in advance to request a consent form. Completing this form ahead of time will help the pharmacist plan accordingly and ensure sufficient quantities and types of vaccines are available.

What to Bring:

• Completed consent form (if received in advance)

• Photo ID

• Insurance card
  Vaccines will be billed directly to your insurance provider. No co-pay is required.

Walk-ins are welcome. Consent forms will also be available onsite for those who have not submitted one in advance.

Vaccines Available:

• Flu – Annual vaccination

• Tdap (Tetanus, Diphtheria, Pertussis) – Booster recommended every 10 years

• Meningococcal – Both B and ACWY formulations available

• HPV – Typically recommended for individuals under age 26

• MMR – For those who may need catch-up doses

• COVID-19 – Eligibility criteria pending updated FDA guidance; more information will be available by clinic date

• Shingrix (shingles)– Recommended for adults aged 50+

• Pneumonia – Recommended for adults aged 50+

• RSV – Recommended for adults 65+, or 50–64 with certain medical conditions
