1095-C Form Availability

Your 1095-C form is available on the portal under the Self-Service tab if you previously consented to receive it electronically.

If you did not provide electronic consent, a paper copy of your 1095-C form has been mailed to the address HR has on file for you.

If you would like to receive your 1095-C electronically next year, please log in to the portal, navigate to Self-Service > Tax Information, and update your preferences to provide electronic consent.
